New Patients
We’re thrilled to meet with you! New patients are encouraged to submit their forms electronically. When you arrive for your appointment, we ask that you bring your insurance card(s), plus any eyewear you use, including sunglasses, computer glasses, contact lenses, or other prescription eyewear.
